    探讨博莱霉素(BLM)致纤维化肺组织HRCT征象与病理学改变以及血管内皮细胞(EC)功能变化间的关系,将健康SD大鼠随机分为实验组和对照组各40只。每只大鼠行HRCT扫描后,检测外周血中vWf水平,并取左下肺叶供制作病理标本。结果实验组大鼠外周血血清vWf水平均高于对照组,以3d组为著(P<0.01),以后随病程渐趋下降,但28d组仍高于对照组(P<0.05)。小叶间隔增厚、胸膜增厚、磨玻璃样影、双肺实变影及蜂窝肺优势地分布在肺的中外带,而支气管血管束增粗、结节及微结节影主要分布于肺的内中带。多数磨玻璃样影和实变影出现在3d和7d组,部分出现在14d组。多数的小叶间隔增厚、支气管血管束增粗及胸膜增厚见于7d以后。胸膜下弧线影在各组中的分布无差异。蜂窝肺均出现在14d组和28d组,以28d组为主。说明PF不同的临床病程有不同的病理改变,HRCT能较准确反应PF的病理变化,并与PF病程有一定的相关性。EC损伤与PF发生发展过程密切相关。

    Abstract:

    To discuss the HRCT characteristics of pulmonary fibrosis(PF)induced by bleomycin and its relationship with the changes in pathology and hemoangioendothelial function, healthy SD mices were divided randomly into experimental group and control group with 40 mice in each group. The serum vWf level was determined after the scan of HRCT in very mice. It is Resulted that serum vWf level in experimental groups was higher than it in control groups, especially in 3 d group (p<0.01), later then decreased gradually with the recovery of the disease. But until 28 d group, it was still higher that it in the control (p<0.05). Interlobular septa thickening, pleural thickening, ground glass opacity, consolidation pattern and honeycombing pattern were mainly distributed in the middle-outside zone of the lung. While the bronchovascular bundles thickening, nodular and micronodular pattern were mainly located in the middle-inner zone of the lung. Most of the ground glass opacity and pleural thickening appeared in 3 d and 7 d group, partially in 14 d group. Most of the interlobular septa thickening, bronchovascular bundles thickening and pleural thickening happened after 7 days. All of Honeycombing lung were in 14 d group and 28 d group, mainly in 28 d group. It is conclused that the pathological changes were different in different period in PF. HRCT can reflect the pathological changes of RF correctly and related to the progress of the disease in some extent. The damage of EC was closely related with the development of PF.
